Nitrendipine is a dihydropyridine calcium channel blocker indicated for the treatment of arterial hypertension. It acts primarily as a vasodilator by inhibiting the influx of extracellular calcium through L-type calcium channels in vascular smooth muscle and myocardial cells, leading to decreased intracellular calcium, relaxation of vascular smooth muscle, reduced peripheral resistance, and lower blood pressure. Unlike some other calcium channel blockers, nitrendipine does not reduce glomerular filtration rate and is mildly natriuretic. It has also been found to act as an antagonist at the mineralocorticoid receptor (antimineralocorticoid). Nitrendipine may have additional neuroprotective effects due to its ability to penetrate the blood-brain barrier and block neuronal L-type calcium channels; it has been studied for potential benefits in dementia prevention by reducing amyloid-beta production in preclinical models[1][4][5][8].
