Overview

Nivocasan is an orally active, irreversible small molecule inhibitor of caspases, specifically targeting caspase-1, caspase-8, and caspase-9. It was originally discovered by LG Life Sciences and later developed by Gilead Sciences. Nivocasan acts as a hepatoprotectant by suppressing unwanted apoptosis and inflammatory responses through inhibition of the activation of key inflammatory cytokines such as interleukin-1β (IL‑1β) and interleukin‑18 (IL‑18). The drug has been investigated in Phase II clinical trials for the treatment of chronic hepatitis C infection, nonalcoholic steatohepatitis (NASH), hepatic fibrosis, and related liver diseases. Its development was ultimately suspended before approval[1][3][5][6].
