This therapeutic regimen is a chemo-immunotherapy combination consisting of the PD-1 checkpoint inhibitor nivolumab and the standard-of-care R-CHOP chemotherapy (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone). It is primarily investigated for the treatment of aggressive B-cell malignancies, including diffuse large B-cell lymphoma (DLBCL), transformed follicular lymphoma (tFL), and high-grade B-cell lymphoma (HGBL). The regimen works through a multi-pronged approach: nivolumab blocks the PD-1 receptor on T cells to restore anti-tumor immune responses; rituximab targets the CD20 antigen on B cells to induce cell lysis; and the CHOP components provide cytotoxic effects by alkylating DNA, inhibiting topoisomerase II, and disrupting microtubule formation. This combination aims to improve outcomes in patients with untreated aggressive B-cell lymphomas by leveraging both direct cytotoxicity and immune system activation.
