Overview

This is a four-drug **combination regimen** of nivolumab, daratumumab, pomalidomide, and dexamethasone investigated for the treatment of **relapsed/refractory multiple myeloma (RRMM)**. - **Nivolumab** is a monoclonal antibody targeting PD-1 (programmed death-1) receptor, blocking its interaction with PD-L1/L2 and thereby promoting anti-tumor immune responses. - **Daratumumab** is a monoclonal antibody directed against CD38 on plasma cells, leading to their destruction via antibody-dependent cellular cytotoxicity, complement-dependent cytotoxicity, antibody-dependent cellular phagocytosis, and direct apoptosis. - **Pomalidomide** is an immunomodulatory small molecule that stimulates T-cell and NK-cell mediated cytotoxicity, and inhibits angiogenesis. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid used for its anti-inflammatory, immunosuppressive, and anti-myeloma effects. The combination aims for synergistic effects: daratumumab and pomalidomide have proven efficacy in RRMM, and adding nivolumab may enhance immune-mediated anti-myeloma activity by overcoming resistance associated with PD-1 axis[4][2]. This regimen has been studied in randomized phase I/II clinical trials for RRMM.
