The NICE regimen is a chemo-immunotherapy combination consisting of the PD-1 checkpoint inhibitor nivolumab and the standard salvage chemotherapy ICE (ifosfamide, carboplatin, and etoposide). Developed and evaluated by the City of Hope Medical Center, this regimen is used as a response-adapted second-line therapy for patients with relapsed or refractory classical Hodgkin lymphoma. The treatment strategy typically involves initial cycles of nivolumab monotherapy followed by the addition of ICE chemotherapy for patients who do not achieve a complete metabolic response, serving as a highly effective bridge to autologous hematopoietic cell transplantation (AHCT). Nivolumab works by blocking the PD-1 receptor on T-cells, while the ICE components provide synergistic cytotoxic effects through DNA alkylation and topoisomerase II inhibition.
