Overview

This combination therapy consists of two immune checkpoint inhibitors, nivolumab (anti-PD-1) and ipilimumab (anti-CTLA-4), administered alongside the immunosuppressants sirolimus (mTOR inhibitor) and prednisone (glucocorticoid receptor agonist). This specific regimen is being investigated by the National Cancer Institute (NCI) in a Phase 2 clinical trial (NCT05896839) specifically for kidney transplant recipients who have developed unresectable or metastatic cutaneous cancers, such as melanoma, Merkel cell carcinoma, basal cell carcinoma, and cutaneous squamous cell carcinoma. The rationale is to leverage the anti-tumor activity of the checkpoint inhibitors while using sirolimus and prednisone to prevent rejection of the transplanted kidney, a common complication when transplant patients receive immunotherapy.
