A multi-drug combination regimen consisting of **nivolumab** (a PD-1 immune checkpoint inhibitor monoclonal antibody), **oxaliplatin** (a platinum-based cytotoxic chemotherapeutic), **leucovorin** (folinic acid, an enhancer of fluorouracil activity), and **fluorouracil** (a fluoropyrimidine antimetabolite), used in the treatment of advanced or metastatic gastric cancer, gastroesophageal junction cancer, and esophageal adenocarcinoma. Nivolumab enhances antitumor immune response by blocking PD-1 on T cells, oxaliplatin induces DNA crosslinks leading to apoptosis, fluorouracil inhibits thymidylate synthase to disrupt DNA synthesis, and leucovorin potentiates the effect of fluorouracil. The combination is given intravenously, typically every two weeks (mFOLFOX6 protocol), and may confer improved overall survival compared to chemotherapy alone[3][7][9].
