Overview

**Nivolumab + poly-ICLC** is a combination investigational regimen consisting of the anti-PD-1 monoclonal antibody nivolumab and the synthetic dsRNA immunostimulant poly-ICLC (polyinosinic–polycytidylic acid–poly-L-lysine carboxymethylcellulose). Nivolumab is an immune checkpoint inhibitor that targets and blocks the PD-1 receptor on T cells, enhancing the immune system's ability to attack tumors. Poly-ICLC is a TLR3 agonist and mimics viral double-stranded RNA, acting as an immunological adjuvant to stimulate dendritic cells and promote in situ vaccination effects within the tumor microenvironment. This combination has been investigated in early-phase clinical trials for unresectable hepatocellular carcinoma (HCC) using a protocol that includes intratumoral and intramuscular administration of poly-ICLC together with intravenous nivolumab. Preclinical and early clinical evidence suggests combined immunostimulation may provide synergistic antitumor activity with manageable safety profiles, though further studies are ongoing[1][2].
