Overview

Nivolumab + sunitinib is a combination regimen of two pharmaceutical agents: nivolumab, a monoclonal antibody immune checkpoint inhibitor targeting the programmed death-1 (PD-1) receptor, and sunitinib, a small molecule multi-targeted receptor tyrosine kinase inhibitor (TKI) with antiangiogenic and antitumor properties. This combination is investigated primarily in advanced or metastatic cancers, including renal cell carcinoma and soft tissue sarcomas, based on the rationale that simultaneous blockade of immunosuppression (via PD-1) and angiogenesis (via VEGF and other kinases) could enhance antitumor efficacy. Nivolumab enhances antitumor immunity by preventing PD-1-mediated inactivation of T cells, whereas sunitinib inhibits multiple kinases involved in tumor angiogenesis and tumor growth. Clinical trials have shown meaningful antitumor activity for the combination but also a comparatively high incidence of grade 3/4 adverse events, with toxicity limiting further development at approved monotherapy doses in some tumor types[3][5].
