Nivolumab + trastuzumab imbotolimod is a combination regimen theoretically comprised of three agents: nivolumab, a programmed death-1 (PD-1) immune checkpoint inhibitor; trastuzumab, a monoclonal antibody that targets the human epidermal growth factor receptor 2 (HER2); and imbotolimod, which is described elsewhere as a TLR9 (Toll-like receptor 9) agonist immunomodulator. As of the current date and per available sources, there is no evidence in the literature, clinical trial registries, or regulatory sources for any clinical-stage combination formally comprising all three agents together. Nivolumab has demonstrated efficacy in combination with trastuzumab and chemotherapy for HER2-positive advanced gastroesophageal adenocarcinoma, as seen in the INTEGA trial, but imbotolimod is not included in these regimens. Mechanistically, such a triple combination would aim to synergistically enhance antitumor immune responses by blocking PD-1-mediated immunosuppression, targeting HER2-positive cancer cells, and activating innate immunity via TLR9 agonism.
