Nivolumab-IRDye800CW (nivo800) is an investigational molecular imaging agent consisting of the monoclonal antibody nivolumab conjugated to the near-infrared fluorescent dye IRDye800CW. Developed by researchers at the Vanderbilt-Ingram Cancer Center, it is designed to assist in the intraoperative visualization of head and neck squamous cell carcinoma (HNSCC). By targeting Programmed Cell Death Protein 1 (PD-1) expressed on tumor-infiltrating lymphocytes and potentially the tumor cells themselves, nivo800 allows surgeons to identify tumor margins and metastatic lymph nodes using fluorescence-guided surgery. This approach aims to improve the precision of surgical resection and ensure clearer margins while sparing healthy tissue. In clinical trial settings, it is often administered following a dose of unlabeled nivolumab to optimize imaging contrast.
