Overview

NKG2D chimeric antigen receptor (CAR) T cells are an adoptive cellular immunotherapy developed by the Fundacion Para La Investigacion Biomedica Del Hospital Universitario La Paz. This therapy involves engineering a patient's or donor's T cells to express a CAR that incorporates the full-length human NKG2D (Natural Killer Group 2 Member D) receptor, typically fused to the CD3-zeta signaling domain and a costimulatory domain such as 4-1BB. Unlike traditional CAR-Ts that target a single antigen (like CD19), NKG2D CAR-Ts recognize a broad range of stress-induced ligands, including MICA, MICB, and ULBP1-6, which are frequently overexpressed on the surface of various hematologic malignancies and solid tumors while remaining largely absent from healthy tissues. This approach is being investigated primarily for pediatric patients with relapsed or refractory acute lymphoblastic leukemia (B-ALL and T-ALL), acute myeloid leukemia (AML), and certain solid tumors like neuroblastoma and osteosarcoma.
