NMS-173 is a potent, covalent, second-generation dual inhibitor of isocitrate dehydrogenase 1 (IDH1) and isocitrate dehydrogenase 2 (IDH2). It was fully discovered and developed by Nerviano Medical Sciences for the treatment of cancers with IDH1 or IDH2 mutations. Unlike first-generation FDA-approved drugs that inhibit only one isoform, NMS-173 targets both IDH1 and IDH2 enzymes, potentially overcoming resistance mechanisms seen with earlier agents. Preclinical studies have demonstrated its ability to decrease intracellular levels of the oncometabolite 2-hydroxyglutarate (2-HG), induce cell differentiation, and inhibit tumor growth in models of solid tumors such as cholangiocarcinoma, low-grade gliomas, chondrosarcoma, as well as in acute myeloid leukemia models. The drug is orally available and has shown statistically significant anti-tumor activity in aggressive cancer models[1][4][6].
