NN1177 (NNC9204-1177) is a long-acting, investigational dual agonist targeting both the glucagon receptor and the glucagon-like peptide 1 (GLP-1) receptor. Developed for weight management in adults with overweight or obesity, it was designed to induce clinically meaningful weight loss by co-activating these two metabolic pathways. Preclinical and early clinical studies demonstrated that NN1177 produced dose-dependent reductions in body weight and liver fat, outperforming semaglutide in some hepatic endpoints at equivalent weight loss. The drug has a higher affinity for GLP-1 receptors than for glucagon receptors (2–4 fold). Despite promising efficacy data, development was discontinued due to safety concerns observed during phase 1 trials—including increased heart rate, decreased reticulocyte count, elevated inflammatory markers (CRP and fibrinogen), hepatic disturbances (elevated AST/ALT), impaired glucose tolerance at higher doses, and reduced blood levels of certain amino acids[1][4][8].
