NNC2215 is a glucose-responsive insulin (GRI) analogue developed by Novo Nordisk for the treatment of Type 1 and Type 2 diabetes. It is engineered to function as a molecular switch that adjusts its bioactivity based on ambient glucose concentrations. The molecule consists of an insulin analogue conjugated to a glucose-binding macrocycle and a glucoside. When blood glucose levels are high, the macrocycle binds to glucose, inducing a conformational change that increases the molecule's affinity for the insulin receptor (demonstrating a 3.2-fold increase in affinity as glucose levels rise from 3 mM to 20 mM). This mechanism allows for automated glycemic control, providing potent glucose lowering during hyperglycemia while reducing the risk of hypoglycemia by decreasing activity when glucose levels are low. NNC2215 is currently in Phase 1 clinical development.
