Overview

No-carrier-added lutetium-177 (n.c.a. Lu-177) is a high-purity radioisotope produced by the neutron irradiation of enriched ytterbium-176, followed by chemical separation. This production route is distinct from the carrier-added method as it yields Lu-177 with high specific activity and eliminates the long-lived radioactive impurity lutetium-177m, which simplifies clinical logistics and waste management. As a beta-emitting radionuclide with a half-life of approximately 6.6 days, it serves as the active pharmaceutical ingredient or precursor in targeted radionuclide therapies (TRT). It is chemically conjugated to targeting molecules, such as peptides (e.g., PSMA-617, DOTATATE) or monoclonal antibodies, to deliver localized ionizing radiation to malignant cells. Its primary applications include the treatment of neuroendocrine tumors and metastatic castration-resistant prostate cancer.
