NO-ferroheme albumin is an experimental therapeutic complex composed of nitric oxide-bound ferroheme (NO-ferroheme) associated with albumin. It is being investigated for its potential to mitigate the vasculopathic and inflammatory effects of cell-free hemoglobin and ferric heme, which are elevated in conditions such as sickle cell disease (SCD) and sepsis. The complex functions by converting pro-oxidative plasma free heme into a stable, vasodilatory form that activates soluble guanylate cyclase (sGC). In preclinical models of LPS-mediated acute lung injury, NO-ferroheme albumin has demonstrated the ability to reduce neutrophil extravasation and the expression of pro-inflammatory cytokines like IL-6, CXCL9, and CXCL10.
