Nocturnal normobaric hyperoxia (NNBH) is a therapeutic intervention being investigated by Beth Israel Deaconess Medical Center for the treatment of retinal ischemia-related conditions, such as diabetic macular edema and retinal vein occlusion. The therapy involves the administration of supplemental oxygen (typically 40% FiO2 at a flow rate of 5 liters per minute) using a home oxygen concentrator while the patient sleeps for 6 to 8 hours per night. The physiological rationale is to increase oxygen delivery to the inner retina from the highly oxygenated choroidal circulation, bypassing the compromised retinal vasculature. This approach aims to reduce retinal hypoxia, which is a primary driver of vascular endothelial growth factor (VEGF) production and subsequent edema, potentially serving as an alternative or adjunct to intravitreal anti-VEGF injections.
