Overview

Nogapendekin alfa is a human interleukin 15 (IL-15) superagonist, specifically an IL-15N72D variant, used as part of a fixed-dose combination with inbakicept (marketed as Anktiva) for the treatment of Bacillus Calmette-Guerin (BCG)-unresponsive non-muscle invasive bladder cancer (NMIBC) with carcinoma in situ (CIS), with or without papillary tumors. It is administered intravesically and always used alongside BCG. Nogapendekin alfa acts as an IL-15 receptor agonist, mimicking the biological actions of native IL-15 but with increased activity and longer half-life due to its N72D mutation. The drug stimulates proliferation and activation of natural killer cells, CD8+ T cells, and memory T cells without expanding immunosuppressive regulatory T cells. This immune activation leads to anti-tumor effects within the bladder wall[1][2][4][5][6][7].
