nogapendekin alfa inbakicept + Tri-Ad5 vaccine is a combination immunotherapy being investigated for the prevention of colorectal and other neoplasms in patients with Lynch syndrome. The combination consists of nogapendekin alfa inbakicept (N-803), a first-in-class interleukin-15 (IL-15) superagonist, and Tri-Ad5, a trivalent recombinant adenovirus-5 (Ad5) vaccine. Nogapendekin alfa inbakicept is a fusion protein comprising an IL-15 mutant (IL-15N72D) bound to an IL-15 receptor alpha (IL-15Rα) Su/Fc fusion protein; it acts as an IL-15 receptor agonist to stimulate the proliferation and activation of natural killer (NK) cells and CD8+ T cells. The Tri-Ad5 vaccine targets three tumor-associated antigens: carcinoembryonic antigen (CEA), mucin-1 (MUC1), and brachyury, which are often overexpressed in precancerous and cancerous cells. By combining the antigen-specific immune priming of Tri-Ad5 with the potent immune-enhancing properties of N-803, the therapy aims to establish a robust immune surveillance mechanism to identify and destroy emerging neoplastic cells in high-risk individuals. This combination is currently being evaluated in a Phase IIb clinical trial sponsored by the National Cancer Institute (NCI).
