Nomegestrol is a synthetic progestogen and a derivative of 19-norprogesterone. It acts as a highly selective agonist of the progesterone receptor, with minimal affinity for other steroid receptors, thereby lacking significant androgenic, estrogenic, or glucocorticoid effects. It is primarily used in hormonal contraception (often in combination with estradiol) and in hormone replacement therapy for the treatment of menopausal symptoms and gynecological disorders such as dysmenorrhea or endometriosis. Beyond its reproductive health applications, nomegestrol has been investigated for its potential neuroprotective and immunomodulatory effects in multiple sclerosis, specifically to prevent postpartum relapses, although clinical trials in this indication have not demonstrated efficacy.
