Overview

Non-enteric coated pancreatic enzymes are a porcine-derived biologic therapy being investigated by the Asian Institute of Gastroenterology for the management of pain in patients with chronic pancreatitis. Unlike standard enteric-coated preparations designed for malabsorption, this formulation is intended to release high doses of proteases (specifically 30,000 units) in the duodenum. The mechanism involves the negative feedback inhibition of cholecystokinin (CCK) receptors, which reduces pancreatic stimulation and intraductal pressure, thereby alleviating pain. This approach is being evaluated in the NE-PERT (Non-enteric Coated Enzymes for Pain in Chronic Pancreatitis) clinical trial (NCT05042284).
