Nonacog beta pegol is a glycoPEGylated recombinant coagulation factor IX (FIX) with an extended half-life, developed for the treatment and prevention of bleeding episodes in patients with hemophilia B (congenital factor IX deficiency or Christmas disease)[1][4][5]. The drug is produced by site-specific attachment of a 40 kDa polyethylene glycol (PEG) moiety to the activation peptide of recombinant FIX. Upon activation by coagulation factors IXa and VIIa during the clotting cascade, the PEG-containing peptide is cleaved off, leaving activated FIX that functions like endogenous FIXa[1][4]. Activated FIX then works in concert with factor VIIIa to activate factor X, triggering thrombin generation and fibrin clot formation[1]. Nonacog beta pegol has demonstrated efficacy for on-demand treatment of bleeds as well as prophylaxis and perioperative management in both adults and children with hemophilia B[2][4][5]. It offers prolonged hemostatic effect due to its extended half-life compared to standard FIX products. The drug was developed by Novo Nordisk.
