nOPV2 candidate 1 is a live-attenuated oral poliovirus vaccine designed to provide immunity against type 2 poliovirus while minimizing the risk of vaccine-derived paralytic polio (VDPV). It is derived from the Sabin type 2 strain but incorporates specific genetic modifications to enhance stability and prevent reversion to neurovirulence. These modifications include changes to the 5'-untranslated region (UTR) to stabilize attenuation, mutations in the 3D polymerase to reduce recombination and increase replication fidelity, and the relocation of the cis-acting replication element (cre) to the 5'-UTR to further inhibit recombination. Developed as part of the Global Polio Eradication Initiative (GPEI), it aims to replace the traditional Sabin OPV2 in outbreak response. Clinical trials led by Pierre Van Damme at the University of Antwerp evaluated its safety, immunogenicity, and genetic stability, leading to its selection as the primary candidate for further development.
