NOSO-502 is a first-in-class odilorhabdin antibiotic derived from Xenorhabdus bacteria, developed by Nosopharm as part of the GNA NOW consortium for treating multidrug-resistant Gram-negative infections, particularly carbapenem-resistant Enterobacteriaceae (CRE) such as *Escherichia coli*, *Klebsiella pneumoniae*, *Enterobacter* spp., and *Citrobacter* spp. It features a novel mechanism of action that inhibits bacterial protein synthesis by targeting the ribosome, demonstrating potent in vitro activity (MIC50/90 typically 0.5-4 mg/L) against resistant strains including those with ESBL, carbapenemases (KPC, OXA, AmpC), and polymyxin resistance, with rapid bactericidal effects and low resistance potential. NOSO-502 shows efficacy in murine models of sepsis, pneumonia, and urinary tract infections via subcutaneous administration, alongside a favorable preclinical safety profile (no cytotoxicity up to 512 μM, no hERG inhibition).[1][2][3][5][6]
