Novolin 30R + pioglitazone + metformin

This combination therapy consists of **Novolin 30R** (a premixed human insulin), **pioglitazone** (a thiazolidinedione), and **metformin** (a biguanide). The treatment strategy involves titrating Novolin 30R to achieve euglycemia, followed by a sequential discontinuation of the insulin, then pioglitazone, and finally metformin, to assess long-term glycemic remission. Novolin 30R provides both rapid-acting (30% regular insulin) and intermediate-acting (70% NPH insulin) components to manage postprandial and basal glucose levels. Pioglitazone improves insulin sensitivity by activating the peroxisome proliferator-activated receptor gamma (PPAR-gamma), while metformin reduces hepatic glucose production and improves peripheral glucose uptake, primarily through the inhibition of mitochondrial complex I.
