Drug intelligence / Profile preview
NOX66 is a rectal suppository formulation of the small molecule idronoxil, developed to enhance its bioavailability and anti-cancer activity. Idronoxil acts primarily by binding to the tumor-specific enzyme external NADH oxidase type 2 (ENOX2), leading to reduced sphingosine-1-phosphate (S1P) and increased ceramide levels in cancer cells, thereby promoting apoptosis. It also inhibits key pro-survival pathways such as phosphoinositide 3-kinase/protein kinase B (PI3K/Akt) signaling and sphingosine kinase, blocks DNA repair mechanisms in cancer cells, and increases natural killer (NK) cell activity. These actions sensitize tumors to chemotherapy and radiotherapy while potentially overcoming resistance mechanisms. NOX66 has been studied as monotherapy and in combination with carboplatin, radiation therapy, immune checkpoint inhibitors like nivolumab, and radionuclide therapy such as lutetium PSMA-617 for various refractory solid tumors including metastatic castrate-resistant prostate cancer[1][3][5][6][7][8].
