**NP137 + pembrolizumab** is an investigational combination therapy under clinical evaluation for advanced or metastatic solid tumors, particularly advanced/metastatic uterine (endometrial) and cervical carcinomas. NP137 is a first-in-class humanized monoclonal antibody that targets netrin-1, a secreted laminin-related protein implicated in cancer cell survival, progression, and resistance by promoting epithelial-to-mesenchymal transition (EMT) and shaping the tumor microenvironment. Pembrolizumab is an immune checkpoint inhibitor monoclonal antibody that binds to PD-1 on T cells, restoring immune responses against tumors. The combination targets both tumor-intrinsic mechanisms of resistance (via EMT inhibition and effects on netrin-1–UNC5B interaction) and tumor-extrinsic immunosuppressive pathways, aiming to improve responses to immunotherapy in tumors with primary or acquired resistance to anti-PD-1/PD-L1 blockade.
