Drug intelligence / Profile preview
Nter-FGF23 is the N-terminal peptide fragment of fibroblast growth factor 23 (FGF23), generated by the proteolytic cleavage of intact FGF23 by the endoprotease furin. Historically, FGF23 cleavage was considered an inactivating mechanism that terminates the hormone's biological activity. However, emerging research from Northwestern University Feinberg School of Medicine has demonstrated that Nter-FGF23 is a biologically active endocrine peptide. In preclinical mouse models, Nter-FGF23 overexpression rescues the lethal phenotype of FGF23 knockout mice, restoring survival, normalizing growth, and correcting skeletal defects. Functionally, Nter-FGF23 acts on the kidney to reduce calcitriol levels and renal phosphate reabsorption, thereby promoting phosphaturia, lowering serum phosphate, and protecting against ectopic calcifications and nephrocalcinosis. This peptide represents a novel therapeutic candidate for the treatment of hyperphosphatemia, ectopic calcification, and associated bone and mineral disorders.
