Sodium ferric gluconate complex is an intravenous iron replacement therapy. It is a stable macromolecular complex of iron (III) oxide hydrate bonded to sucrose with a chelating gluconate function. Developed by Watson Pharmaceuticals as Nulecit (the first generic alternative to the innovator product Ferrlecit), it is indicated for the treatment of iron deficiency anemia in adult and pediatric patients (6 years and older) with chronic kidney disease (CKD) who are receiving hemodialysis and supplemental epoetin therapy. The drug works by replenishing iron stores, specifically targeting ferritin light chain to maintain iron homeostasis. It delivers iron to the reticuloendothelial system, where it is stored in a soluble, non-toxic form and subsequently released for hemoglobin synthesis and erythropoiesis.
