Nutmeg extract (NEMM) is a topical botanical formulation developed by researchers at the Faculty of Medical Sciences, University of the West Indies, for the treatment of painful diabetic neuropathy (PDN). The preparation consists of a combination of mace oil (2%), nutmeg oil (14%), methyl salicylate (6%), menthol (6%), and coconut oil. It is designed to alleviate neuropathic pain through the analgesic and counter-irritant properties of its components, specifically menthol's action on TRPM8 receptors and methyl salicylate's inhibition of cyclooxygenase (COX) enzymes. In a randomized, double-blind, placebo-controlled trial involving 74 subjects, the extract did not demonstrate significant clinical benefit over a 4-week period compared to preparations containing only menthol and methyl salicylate.
