NVS-PAK1-1 is a selective, orally bioavailable allosteric inhibitor of p21-activated kinase 1 (PAK1). Developed by Novartis, it is being investigated for its potential to treat Alzheimer's disease by preserving synaptic integrity. PAK1 is a key regulator of the actin cytoskeleton via the cofilin pathway; in Alzheimer's disease, its dysregulation and translocation to the cell membrane are associated with the loss of dendritic spines and cognitive decline. Preclinical studies in 5XFAD mouse models have demonstrated that NVS-PAK1-1 can protect dendritic spines from amyloid-beta and tau-induced degradation, restoring spine density to levels comparable to wild-type mice without significantly altering amyloid plaque load. This suggests a mechanism of action focused on conferring resilience to synapses rather than direct amyloid clearance. The compound is also representative of a broader class of PAK1 inhibitors currently being explored in oncology clinical trials.
