Overview

NVX-207 is a **semi-synthetic derivative of betulinic acid** developed as an anticancer agent, notably for dermatological neoplasms in animals and potentially humans. It is a **pentacyclic triterpenoid ester** designed to improve the pharmacological limitations of its parent compound, betulinic acid, most notably increasing water solubility and potency. Mechanistically, NVX-207 **induces apoptosis predominantly via the mitochondrial (intrinsic) apoptotic pathway**, involving activation of caspases-9, -3, and -7, and cleavage of poly (ADP-ribose) polymerase (PARP). It also shows alterations in gene expression associated with lipid metabolism, including upregulation of Insig-1, low-density lipoprotein receptor, and HMG-CoA reductase, and it can bind to apolipoprotein A-I, potentially impacting cholesterol transport. NVX-207 has **demonstrated significant cytotoxicity and antiproliferative effects in vitro** against various human, canine, and equine cancer cell lines, including equine melanoma and equine sarcoid, and has shown in vivo efficacy and good tolerability in animal models. It is mainly investigated as a **topically applied treatment** for **equine melanoma** and other skin neoplasms, where it penetrates into the epidermis and dermis and accumulates above therapeutic concentrations. Clinical results in horses are preliminary, with some tumor volume reduction observed, but larger trials are needed[1][3][4][5][7].
