Overview

NXN-188 is a first-in-class, dual-action small molecule developed for the acute treatment of migraine. It acts as both a selective neuronal nitric oxide synthase (nNOS) inhibitor and a serotonin 5HT1B/1D receptor agonist. By inhibiting nNOS, it reduces nitric oxide production implicated in migraine pathogenesis, while its agonism at 5HT1B/1D receptors provides vasoconstrictive and anti-migraine effects similar to triptans. Some sources also suggest calcitonin gene-related peptide (CGRP) antagonism may contribute to its mechanism[3][5][6][7]. The drug was originally developed by NeurAxon and later licensed to Knight Therapeutics for certain territories[3]. Clinical trials have shown that oral administration of NXN-188 can provide pain relief in acute migraine attacks, with some evidence of efficacy on multiple endpoints such as pain freedom and reduced use of rescue medication; however, primary endpoints were not always met with statistical significance[6][8].
