NY-ESO-1 T-cell receptor-engineered peripheral blood stem cells represent an investigational adoptive cell therapy approach where autologous CD34+ hematopoietic stem and progenitor cells (HSPCs) are genetically modified with a T-cell receptor (TCR) specific for the cancer-testis antigen NY-ESO-1. Unlike conventional TCR-T therapies that modify mature T-cells, this strategy involves engineering the source stem cells to provide a continuous, long-term supply of NY-ESO-1-specific T-cells as they differentiate and mature within the patient's immune system. The engineered TCR is typically restricted to HLA-A*02:01 and recognizes a specific peptide sequence (SLLMWITQC) from the NY-ESO-1 protein. By utilizing stem cells, the therapy aims to overcome the limitations of limited T-cell persistence and exhaustion often seen in mature cell infusions, potentially establishing a lifelong memory response against NY-ESO-1-expressing tumors such as melanoma and synovial sarcoma.
