NYX-458 is an oral small molecule drug that acts as a positive allosteric modulator of the N-methyl-D-aspartate (NMDA) receptor. It was developed by Aptinyx for the treatment of cognitive impairment associated with Parkinson’s disease and dementia with Lewy bodies. The drug was designed to address NMDA receptor dysregulation resulting from dopamine loss in these neurodegenerative conditions, aiming to improve cognition and daily function. Unlike other NMDA modulators such as ketamine or MK-801, NYX-458 features a novel spiro-β-lactam-based chemical structure and was inspired by rapastinel, another NMDA partial agonist. Despite promising preclinical results showing pro-cognitive and antidepressant effects, clinical development was discontinued after Phase 2 trials failed to demonstrate meaningful efficacy over placebo[1][2][4][6].
