The O-acetyl pectin-rEPA conjugate vaccine is an investigational semi-synthetic conjugate vaccine developed by the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) for the prevention of typhoid fever. The vaccine is designed to mimic the Vi capsular polysaccharide of *Salmonella typhi*, which is the primary target for protective immunity. It consists of O-acetylated pectin (OAcPec), a chemically modified plant polysaccharide derived from citrus peel, which is antigenically modified to resemble the bacterial Vi antigen. This polysaccharide is covalently conjugated to a carrier protein, recombinant exoprotein A from *Pseudomonas aeruginosa* (rEPA), to convert the T-cell independent polysaccharide into a T-cell dependent antigen, thereby enhancing immunogenicity and potentially extending protection to young children and infants. Clinical evaluation in Phase 1 trials has demonstrated that the vaccine is safe and capable of eliciting specific IgG antibodies against the Vi polysaccharide.
