Combination of **obefazimod** (formerly ABX464) and **methotrexate** used in clinical trials for the treatment of moderate-to-severe active rheumatoid arthritis in patients with inadequate response to methotrexate or anti-tumor necrosis factor alpha therapy. Obefazimod is a **first-in-class oral small molecule** that upregulates the production of micro-RNA-124 (miR-124), an inhibitor of mRNA that modulates inflammatory and immune pathways by blocking cytokine production. Methotrexate is an antimetabolite antifolate that suppresses inflammation mainly through adenosine signaling and inhibition of multiple enzymes involved in purine metabolism, decreasing T-cell activation and cytokine production. In Phase 2a clinical trials, the combination demonstrated enhanced efficacy (higher ACR20/50 response rates, improved DAS28 scores) and was generally well tolerated at 50 mg obefazimod; gastrointestinal disorders were the most common adverse events leading to discontinuation[1].
