Obidoxime is a small molecule drug belonging to the oxime family, primarily used as an antidote for organophosphate poisoning, including exposure to nerve agents (such as sarin, tabun, VX) and organophosphorus pesticides. Its main mechanism of action is reactivation of acetylcholinesterase (AChE) that has been inhibited by organophosphates. Obidoxime achieves this by having a higher affinity for the phosphate group bound to AChE than the enzyme itself; it removes the phosphate group from AChE, restoring its normal function and allowing elimination of the resulting compound via urine. It is considered more potent than pralidoxime in reactivating AChE inhibited by certain agents. Clinical use often involves combination with atropine to manage cholinergic crisis symptoms during poisoning events[1][2][5][6].
