Obinutuzumab is a humanized type II anti-CD20 monoclonal antibody used primarily in the treatment of chronic lymphocytic leukemia (CLL) and follicular lymphoma. It works by binding to the CD20 antigen on B lymphocytes, leading to enhanced direct cell death and increased antibody-dependent cellular cytotoxicity (ADCC) and phagocytosis (ADCP), with reduced complement-dependent cytotoxicity compared to earlier anti-CD20 antibodies like rituximab. Obinutuzumab is glycoengineered for improved efficacy. Methylprednisolone is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressive properties; it modulates gene expression via the glucocorticoid receptor, reducing inflammation and immune responses. The combination of obinutuzumab with high-dose methylprednisolone has been investigated as a chemotherapy-free regimen for CLL, leveraging synergistic effects between corticosteroids and anti-CD20 antibodies[5][6][8].
