Overview
**Oblimersen + fludarabine + cyclophosphamide** is a three-drug combination regimen primarily investigated in relapsed or refractory chronic lymphocytic leukemia (CLL). Oblimersen is an antisense oligonucleotide targeting Bcl-2 mRNA to reduce Bcl-2 protein expression, thereby enhancing apoptosis in cancer cells. Fludarabine is a purine analog that inhibits DNA synthesis and repair, while cyclophosphamide is an alkylating agent that causes DNA crosslinking and cell death. The combination aims to overcome chemotherapy resistance mediated by Bcl-2 and improve response rates and survival compared to standard chemotherapy. Clinical trials have shown that this regimen significantly increases complete and nodular partial response rates, with extended response duration and improved survival in patients with fludarabine-sensitive disease[1][2][4][5][7].
