Octaplex

Octaplex is a human plasma-derived, four-factor prothrombin complex concentrate (PCC) containing the vitamin K-dependent coagulation factors II, VII, IX, and X, as well as the anticoagulant proteins C and S. It is manufactured by Octapharma and is primarily indicated for the rapid reversal of acquired coagulation factor deficiency, such as that caused by vitamin K antagonist (e.g., warfarin) therapy. In the context of the FiiRST-2 (Factors in Initial Resuscitation of Severe Trauma 2) trial, sponsored by the University Health Network in Toronto, Octaplex is being evaluated as an early hemostatic therapy for patients with traumatic hemorrhage and trauma-induced coagulopathy. In this trial, it is administered in combination with fibrinogen concentrate (Fibryga) to restore thrombin generation and achieve rapid hemostasis, serving as an alternative to traditional frozen plasma-based resuscitation protocols.
