Overview

Octenidine is a cationic surfactant and antiseptic agent used primarily for topical disinfection of skin, mucous membranes, wounds, and in oral care. It exhibits broad-spectrum antimicrobial activity against Gram-positive and Gram-negative bacteria as well as fungi. Its mechanism of action involves binding to phospholipids and lipopolysaccharides in microbial cell membranes, leading to membrane disruption and cell death[1][2][5]. Octenidine is not systemically absorbed when applied topically or on mucosa[5]. It has been widely used in Europe since the late 1980s as an alternative to chlorhexidine due to its efficacy and low toxicity profile[7]. Clinical uses include preoperative skin preparation, wound antisepsis, oral rinses for periodontitis/gingivitis prevention, Staphylococcus aureus decolonization (including MRSA), catheter site care, and prevention of nosocomial infections such as surgical site infections[6][7][8].
