Odronextamab + chemotherapy is an investigational combination therapy being studied primarily for the treatment of previously untreated diffuse large B-cell lymphoma (DLBCL) and follicular lymphoma. Odronextamab is a fully human, IgG4-based bispecific monoclonal antibody that binds CD20 on malignant B cells and CD3 on cytotoxic T cells, physically bridging these cell types to enable direct T cell–mediated cytotoxicity against CD20+ B cells. This mechanism is distinct from classic monoclonal antibodies, providing a targeted immunotherapy approach by activating T cells independently of their conventional antigen recognition pathways. The chemotherapy component varies but typically refers to conventional cytotoxic agents used in standard lymphoma regimens. Clinical trials are investigating the efficacy and safety of odronextamab in combination with chemotherapy, compared to the established regimen of rituximab plus chemotherapy, especially in patient populations with relapsed or refractory disease[1][3][7][8].
