Overview

This is a combination regimen consisting of three distinct therapeutic modalities for the treatment of B-cell malignancies, especially relapsed or refractory diffuse large B-cell lymphoma (DLBCL): - **Odronextamab** is a bispecific monoclonal antibody that targets CD20 on B-cells and CD3 on T-cells, facilitating T-cell-mediated cytotoxicity of malignant B cells. - **Loncastuximab tesirine** is an antibody-drug conjugate (ADC) composed of a CD19-directed humanized monoclonal antibody conjugated to the cytotoxic pyrrolobenzodiazepine dimer toxin, designed to selectively kill CD19-positive B-cells. - **CAR T-cells** (Chimeric Antigen Receptor T-cells, particularly CD19-directed variants such as axi-cel, tisa-cel, or liso-cel) are genetically engineered autologous T-cells targeting CD19, leading to direct killing of B-cells expressing CD19. Each agent targets the B-cell population via different mechanisms and at different antigens, offering potential additive or synergistic effects. This multimodal approach is being studied or used in heavily pretreated DLBCL, especially after failure of CAR T-cell therapy, or in situations requiring novel combinatorial approaches[2][3][4][6].
