ofloxacin + ornidazole is a fixed-dose combination antibiotic and antiprotozoal medication. Ofloxacin is a second-generation fluoroquinolone that exerts its antibacterial effect by inhibiting bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination. Ornidazole is a nitroimidazole derivative that, upon reduction within the target cell, forms reactive intermediates that interact with DNA, leading to the loss of helical structure and strand breakage, which inhibits protein synthesis and causes cell death in susceptible anaerobic bacteria and protozoa. This combination is primarily indicated for the treatment of mixed infections caused by aerobic and anaerobic bacteria or protozoa, including acute diarrhea, dysentery, gynecological infections, and respiratory tract infections.
