Overview

OK-432 + cyclophosphamide is a combination therapy that has been studied for treating various types of cancer, particularly malignant lymphoma and leukemia. This combination represents an immuno-chemotherapy approach, where OK-432 (a streptococcal preparation) provides immunotherapeutic effects while cyclophosphamide serves as a chemotherapeutic agent.\n\n## Therapeutic Applications\n\nClinical trials conducted between 1972 and 1977 evaluated this combination for treating malignant lymphoma patients. The study compared patients receiving chemotherapy alone (control group) with those receiving chemotherapy plus OK-432 (treated group). The treated group showed higher remission rates and longer remission duration and survival, especially in patients with Hodgkin's disease, though the difference wasn't statistically significant due to the limited number of cases[1].\n\nIn animal studies, this combination therapy has shown promising results. When used to treat mice bearing syngeneic tumors like leukemia P388, the synergistic effects of cyclophosphamide were potentiated by OK-432 in a time- and dose-dependent manner[2][5]. Some mice were even cured by this three-agent treatment and developed immunity against later challenges with the same tumor cells[2].\n\n## Mechanism of Action\n\nThe combination works through complementary mechanisms:\n\n**OK-432 (Picibanil):**\n- A streptococcal preparation that enhances immune response\n- Increases the activity of cytotoxic spleen cells against tumor cells\n- Potentiates syngeneic and semi-syngeneic transplantation resistance\n- Augments primary and secondary cytotoxicity mediated by spleen cells\n\n**Cyclophosphamide:**\n- An alkylating agent that interferes with cancer cell growth\n- Belongs to the group of cancer medicines called alkylating agents\n- Used to treat various cancers including those of the ovaries, breast, blood and lymph system\n\nThe combination appears particularly effective because OK-432 can reverse the immunosuppressive effects of chemotherapy. While administration of adriamycin and cyclophosphamide together can inhibit tumor transplantation resistance and in vitro cytotoxicity, this inhibition can be reversed by injection of OK-432[2].\n\n## Safety Considerations\n\nCyclophosphamide may affect normal body cells, causing side effects that range from serious to cosmetic (like hair loss). It should not be used with certain live vaccines, and drug interactions should be carefully monitored[4].\n\nFor patients with specific conditions like severe lung dysfunction, lung fibrosis, history of hypersensitivity to bleomycin or similar compounds, severe renal dysfunction, severe heart disease, or history of radiation to the chest, special caution is needed when using these therapies[3].\n\nThis combination represents an early example of immuno-chemotherapy, a treatment approach that continues to evolve in modern cancer care.
