OKI-345 is a next-generation, small molecule pan-mutant selective inhibitor of Phosphatidylinositol 3-kinase alpha (PI3Kα) being developed by OnKure Therapeutics. It is designed to selectively inhibit all major hotspot mutations of PI3Kα while sparing the wildtype enzyme, thereby potentially increasing the therapeutic index and reducing class-related toxicities such as hyperglycemia. The drug is intended for the treatment of breast cancer and is engineered for low drug-drug interaction potential to facilitate combination with standard-of-care therapies. OnKure plans to file an Investigational New Drug (IND) application for OKI-345 in the first half of 2027.
