Overview

Olanzapine + quetiapine is a combination of two atypical antipsychotic drugs, each with distinct but overlapping pharmacological profiles. Both agents are primarily used in the management of schizophrenia and bipolar disorder. Olanzapine acts mainly as an antagonist at dopamine D2 and serotonin 5-HT2A receptors, with additional antagonism at histamine H1 and muscarinic receptors, contributing to its antipsychotic effects as well as side effects such as weight gain and sedation. Quetiapine also antagonizes dopamine D2 and serotonin 5-HT2A receptors but has a different receptor binding profile, including significant activity at histamine H1 and adrenergic alpha1 receptors. Both drugs are metabolized by different cytochrome P450 enzymes (olanzapine by CYP1A2/CYP2D6; quetiapine by CYP3A4). The combination is not standard clinical practice due to increased risk of additive side effects (e.g., sedation, anticholinergic burden) without clear evidence for superior efficacy over monotherapy[1][2][4][5].
