Olaparib + paclitaxel is a combination therapy consisting of olaparib, an oral poly (ADP-ribose) polymerase (PARP) inhibitor, and paclitaxel, a microtubule-stabilizing chemotherapeutic agent. Olaparib inhibits PARP1 and PARP2 enzymes involved in DNA damage repair, exploiting tumor cells' deficiencies in homologous recombination repair—particularly those with BRCA mutations—to induce synthetic lethality and cell death. Paclitaxel disrupts microtubule dynamics required for mitosis, leading to cell cycle arrest and apoptosis. This combination has been investigated primarily for advanced gastric cancer after first-line therapy failure and has also been explored in breast cancer settings. In the phase III GOLD trial for advanced HER2-negative gastric cancer, the combination did not meet its primary endpoint of overall survival improvement compared to paclitaxel alone but showed a similar safety profile[1][2][7]. The regimen is under continued investigation for biomarker-enriched populations.
